Milwaukee, WI as an Oncology Care Manager.
You will be responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost-efficient care for an oncology patient caseload. You will function as the main advocate and contact person for patient, family, team members, employees and outside agencies to assure continuity of care and optimal clinical and resource outcomes.
As the Oncology Care Manager with CTCA, you will be expected to:
* Assess the patient/family needs related to the medical diagnosis, treatment providers/options, financial resources, discharge and psychosocial needs of our patients
* Collaborate with health care team members to develop a multidisciplinary plan of care and set patient-centered goals for patient/families; provide oversight and direction to LPN team members
* Facilitate and/or participate in conferences and rounds that provide ongoing evaluation of interdisciplinary intervention, goal attainment and treatment plan revision
* Provide ongoing communication with patients, families, medical staff, care givers and third-party payers, to include utilization assessment for pre-certification and continued stay justification
* Conduct and reinforce patient education, including: treatment plan, financial implication of care, symptom management, side effects, patient/family responsibilities related to self care, available resources (inside and outside) and discharge plan
* Respond in a timely manner to patient and family needs at home through effective symptom management, coordination of home care services and crisis intervention
Minimum qualifications:
* BSN in Nursing preferred
* Current Illinois RN Licensure
* Minimum of 3 years Oncology nursing experience
* 1 year experience in a nursing management or similar role
* Strong organizational, verbal and written skills
